Gradients: amplifiers, coils, cables and cooling

Locate a gradient fault to the amplifier, its supply, the filters and cables, the gradient coil or its cooling, using the error history, per-axis tests, permitted swaps and safe insulation measurements, and recognise the artefacts that point at the gradient chain.
